ADVISORY 2: Guide to Securing Travel Clearance Sticker
In relation to the travel tips and special advisory on Ebola issued December 11, 2014 (University Issues Holiday Vacation Tips, Advisory on Ebola), Silliman University will be requesting all students, faculty and staff to secure a travel clearance sticker upon entering the gates. This will take effect January 2, 2015 (work to resume January 5, 2015; classes, January 6, 2015) until January 10, 2015.
A travel clearance will be issued in sticker form for pasting to the ID. This is issued by health officers stationed at five designated gates for entrance: Langheim, Villareal, Administration, Laguna and Channon. For the same duration, Gate 7 (Reyes) will be exclusively for exit. Only two questions will be asked:
- Have you traveled to a country where there is an Ebola outbreak?
- Have you been in contact with someone who has traveled to a country known to have Ebola outbreak and who may be manifesting symptoms?
Separate arrangement is made for the Basic Education Department.
Dormers (for those arriving January 2 to 4, 2014)
If arriving between 6AM – 8PM:
Step 1
Proceed to the Information Booth (located across Kross-Kat)
Step 2
Secure travel clearance.
Step 3
(A) Cleared – Issued travel clearance sticker; present to guard
(B) Referral – Accomplish Travel History Form;
[B1] Once cleared – Present travel clearance sticker to guard;
[B2] Referral 2 – Endorsed to University Clinic
If arriving before 6AM or after 8PM, proceed to the dormitory and undertake process stated above with the dormitory manager.

Step 1
Answer questions by the gate.

(A) Cleared – Issued travel clearance sticker; present to guard
(B) Referral – Accomplish Travel History Form at booth near gate

[B2] Referral 2 – Endorsed to University Clinic
Those who have traveled to countries affected with Ebola and entering in vehicles, parking is provided in the Katipunan Hall area, so you can accomplish the necessary steps before proceeding to class.
Asia and the Philippines continue to be Ebola-free. These are some guidelines that we encourage you to observe to protect ourselves from being affected by this deadly virus.
We anticipate that most Silliman students and personnel would not have traveled abroad in the coming holidays, but we will do this procedure anyway to observe good precautionary practices.
